What documents does an animal rescue need from an adopter?
Most rescues need proof of address, written permission from the landlord if the adopter rents, and photos of the home and any garden or fencing. Alongside those documents, the application asks where the animal would live, who else is in the household, what experience the adopter has and how the animal will be cared for, and it ends with the adopter’s consent to a home visit.
- Proof of address: a utility bill, bank or council statement from the last three months (required).
- Landlord permission to keep a pet, only if the adopter rents.
- Photos of the living space and any outdoor area or fencing, as several files.
- Home-visit consent, an agreement to return the animal to you if they can no longer keep it, and a declaration that everything is accurate.
Why adoption applications take so long to get through
- An adopter says their landlord is fine with pets. Nobody asks for it in writing until the day before collection.
- The form comes back as a Word document with the other-pets question left blank, so someone has to email and ask.
- The home-check volunteer turns up without the application, because it is sitting in the coordinator’s inbox.
- Photos of the garden arrive in a messaging app, one at a time, and never make it into the adopter’s file.

Phone, address, home type, own-or-rent, the care plan and all three agreement boxes are required; the rest is optional so an adopter is not stopped by a question that does not apply to them. Accepts photos, PDF, Word and Excel up to 10 MB each.
How it runs for a rescue
Let the reminders chase
Give the link a due date and a reminder interval. Intaky emails an adopter who has not finished every few days, only during working hours in your timezone, and stops after six nudges.
Accept, or ask for one thing
When a garden photo does not show the fence, reject that one item with a note; everything already accepted stays locked. Export the finished application as a PDF with the documents attached for the home-check volunteer.
Questions about adoption applications
What should a pet adoption application form ask?
Contact details and proof of address, the type of home and whether the adopter owns or rents, who lives there including children and other pets, their experience with animals, how long the animal would be alone, a care plan for exercise, feeding and vet care, and who looks after it during holidays. It should end with consent to a home visit and an agreement to return the animal to the rescue if they can no longer care for it. Intaky's pet adoption template asks all of this, and you can change any of it.
How do I check that an adopter who rents is allowed a pet?
Ask for the landlord's written permission as a document on the application. The template asks every adopter whether they own or rent, and includes a landlord permission upload for those who rent. You review the letter yourself; Intaky does not contact the landlord.
How do we stop chasing adopters for missing information?
Give the application link a due date and let the reminders do the chasing. Intaky emails the adopter every few days until the application is complete, only during working hours in your timezone, and stops after six reminders. You can see which applications are finished and which items the others still owe.
Can adopters send photos of their home from their phone?
Yes. The home photos request accepts several files, so the living room, the garden and the fence are one item on the application. Photos, PDF, Word and Excel files are accepted, up to 10 MB each.
What happens if a photo or document is not good enough?
You reject that one item with a note saying what you need, for example a photo that shows the whole garden fence. The adopter sees the note and replaces only that item; everything you have already accepted stays locked.
Can we have separate application forms for dogs, cats and fostering?
Yes. Each form is its own application type with its own questions, documents and public link, so a dog adoption, a cat adoption and a foster application can ask different things from the same workspace.
Does Intaky do the home visit or check an adopter's background?
No. Intaky collects the application, the documents and the adopter's consent to a home visit. Your rescue arranges the visit, carries out any checks you make, and decides who adopts. The PDF export gives your home-check volunteer the whole application in one file.
Does it carry our rescue's name and logo?
Yes. Your logo and colours appear on your own subdomain, on every page the adopter sees.
